| What's Your Number: Patient Cardiovascular Health Assessment
Patients often ask HeartFirst providers their risk of having a heart attack. Being able to quantify a situation with numbers gives us a comfort level. The following questionnaire quantifies your risk based on recommendations of the American Heart Association and the American College of Cardiology. Please take a few minutes to complete and get YOUR NUMBER. If your score is more than 10 in any of these categories, you are at risk for heart disease.
